General Functions: Responsible for the case management services to those living with families/guardians, or in group homes.
Specific Functions: •Ensure that case records are complete and maintained in an orderly manner, ensuring that all required documentation is accurate and provided in a timely manner.
•Conduct formal staffings and other meetings with the Interdisciplinary Team to evaluate client progress and revise the individual training plan. Ensure formal assessment instruments are giving to assigned clients as scheduled by agency policy. Monitor the implementation of behavior plans, suggesting changes as needed.
•Attend and participate in behavior review and program review committee meeting as scheduled.
•Oversee the daily training of the janitorial crew as well as the semi-annual wage and hour ("90/10") reviews. Ensure that time-sheets are turned in on schedule.
•Ensure the timely and accurate preparation of reports requested by the Coordinator.
•Oversee the timely departure of all vehicles at the beginning of the day Howe and Ludeman and also at day's end. Fill monitor vacancies, and find substitutes as needed. Periodically review the condition of vehicles when they return to Joyce Road.
•Participate in Case Record Review/ Program Quality Assessment Committee meetings, special team meetings, and all program meeting as directed.
•Provide input into the performance appraisal process to the supervising Program Manager of program staff who also serve as bus monitors and drivers.
•Facilitate the exit, discharge process and arrangements for follow-up and appropriate supportive services to individuals who terminate from the program.
•Additional duties as assigned.
Qualifications: B.A. in human services field, i.e. Special Education, Rehabilitation, Sociology, Psychology or related field. Minimum one year experience with individuals with developmental disabilities and knowledge/ experience with DOL standards preferred. Must meet qualifications for Qualified Human Services Professional (QHSP).Driving record and auto insurance in accordance with agency policy. A criminal background check and post offer drug screen required . EOE.
